Please print and fill out these forms and submit prior to your first visit. If you are unable to send via fax or email, please bring completed paperwork with you on the day of your visit or we may have to reschedule. Registering on day of visit takes additional time which is not included in your scheduled time for treatment. If you have any additional questions please feel free to contact our Office Manager.
